FDA warns of overdoses from dosing errors with compounded semaglutide vials

The FDA alerted clinicians, compounders and patients to adverse events linked to dosing errors with compounded semaglutide supplied in multiple-dose vials. Some patients drew up five to 20 times the intended dose, and some clinicians miscalculated conversions between milligrams and units, leading to doses five to 10 times too high. Reported harms included nausea, vomiting, fainting, dehydration, acute pancreatitis and gallstones, and some patients were hospitalized.
