Tirzepatide
Dual GIP and GLP-1 receptor agonist
Status
Approved
Approved by the FDA (Mounjaro 2022, Zepbound 2023) and EMA (2022) for type 2 diabetes, weight management and, in the US, obesity-related sleep apnea, with a US cardiovascular-risk indication added in August 2026; the FDA confirmed the shortage resolved on December 19, 2024, and enforcement discretion for compounded copies ended on February 18, 2025 (503A) and March 19, 2025 (503B).
Evidence
Strong
Multiple large phase 3 programs (SURPASS, SURMOUNT) plus a cardiovascular outcomes trial (SURPASS-CVOT) support its approved uses.
Tirzepatide is a once-weekly injected peptide that activates both GIP and GLP-1 receptors. It is approved for type 2 diabetes, chronic weight management and obesity-related sleep apnea, and in the US to lower cardiovascular risk in high-risk type 2 diabetes.
Structure
39 residues- aK20C20 fatty diacid (eicosanedioic acid) via γGlu-2xOEG linker (albumin binding)

What it is used for
- Type 2 diabetes in adults and children 10 and older, glycemic control (approved)
- Chronic weight management in adults (approved)
- Moderate to severe obstructive sleep apnea with obesity (approved)
- Reducing major cardiovascular events in high-risk type 2 diabetes (approved)
- Heart failure with preserved ejection fraction and obesity (studied)
How it works
Tirzepatide is built on the sequence of GIP, a gut hormone, but also activates the GLP-1 receptor. Together these actions increase glucose-dependent insulin release, lower glucagon, slow stomach emptying and reduce appetite and food intake. Aib residues at positions 2 and 13 protect it from breakdown by DPP-4, and a C20 fatty diacid on Lys20 binds albumin, giving a half-life of about five days that allows weekly dosing.
How strong the evidence is
Strong
SURMOUNT-1 (2,539 adults with obesity, no diabetes) found mean weight loss of 15.0% to 20.9% at 72 weeks across 5 to 15 mg versus 3.1% with placebo. SURMOUNT-5 showed greater weight loss than semaglutide 2.4 mg (20.2% vs 13.7%). In type 2 diabetes, SURPASS-2 found larger HbA1c reductions than semaglutide 1 mg, and SURPASS-CVOT (13,165 patients) showed non-inferiority to dulaglutide for major cardiovascular events (12.2% vs 13.1%). SURMOUNT-OSA cut the apnea-hypopnea index by about 20 events per hour more than placebo. SUMMIT studied heart failure with preserved ejection fraction.
Risks and side effects
- Nausea, diarrhea, vomiting and constipation, mostly during dose escalation
- Boxed warning: thyroid C-cell tumors in rats; contraindicated with a personal or family history of medullary thyroid cancer or MEN 2
- Acute pancreatitis, gallbladder disease and acute kidney injury from dehydration
- Hypoglycemia with insulin or insulin secretagogues, and diabetic retinopathy complications in people with prior retinopathy
- Risk of pulmonary aspiration during general anesthesia or deep sedation
Label dosing
Label: start at 2.5 mg once weekly for 4 weeks, then increase in 2.5 mg steps at least 4 weeks apart to a maintenance dose of 5 to 15 mg weekly (maximum 15 mg).
